Certificate of Insurance
In order for a club or recreational league registered with OVA to obtain a certificate of insurance, all requests are processed through the OVA office. Please forward your request to

The following information is required along with your request:
1. Name of the school board(s) & school(s).
2. Name of the contact person.
3. Email address or fax number for the contact person (email preferred if available).
4. Name of the club.
Once this information has been received your request will be processed.
